Referral Information
Thank you for your interest in referring your patient to our endodontic clinic.
In order to ensure that we can offer excellent care for patients referral form (see below) is required.
All referrals will be screened for suitability based on patient’s medical history and complexity of care. Once screened, we will communicate details and requirements with the patients and referring offices.
Please note there are currently no wait times for patients to be seen by our endodontics specialist Dr. Arezu Hanson.
